Tmi, but please help me...5 weeks post partum?

got a major concerning question, this is tmi.. but.. 2 nights ago, i got a brown bloody discharge so i took a shower and ya know put a pad on and went on about my night.. yesterday i had cramping, went for a walk and when i came home the bleeding was light but a bright red color, i didnt think much of it, now last night i went to the bathroom and after i took a shower i was bleeding heavy, not extremely heavy but enough to where i would have to sit on the toilet and it would drip... when gabriella woke up i had time to run to the bathroom and change the pad, pretty soaked from sleeping from just about midnight until almost 3... when i got back to the room made her a bottle sat on the glider with ehr and i felt a huge gush of blood, i put her down to go check myself and there wasa a huge blood clot, it was pretty big, ever since then i been losing a lot of blood and its bright red, it'll slow down pick back up..what could be wrong... sorry for the tmi.. i dont wanna rush to the hospital for nothing if its nothing, help! please...
